This confidential questionnaire helps our ministry team understand your spiritual background, personal history, current struggles, and areas where you are seeking healing.
Completing this form allows us to pray more effectively and prepare appropriately for ministry.

Once we have reviewed your questionnaire, we will schedule a Discovery Session by Zoom or, when available, in person.
During this session we will:
Listen to your story.
Discuss your spiritual journey.
Clarify the areas where you desire healing or freedom.
Answer your questions.
Prayerfully discern the most appropriate ministry pathway.
The Discovery Session is an important step in ensuring that ministry is approached with wisdom, compassion, and biblical discernment.

If healing and/or deliverance ministry is appropriate following the Discovery Session, we will schedule a ministry appointment with one of our trained ministry teams.
Every ministry session is centered on Jesus Christ and guided by Scripture, prayer, the leading of the Holy Spirit, and pastoral care.
Our goal is not merely to address symptoms but to help people experience genuine freedom, healing, and restoration.
